Elder Care Connections of Vermont offers an initial 15 minute complimentary phone consultation to discuss concerns.  From there we can schedule a time to meet with your family and conduct a comprehensive needs assessment.  We’ll utilize this assessment to develop a care plan for your loved one(s) to address all areas of well-being including:
